The way I've always understood the song was based heavily on the lines "Love is watching someone die". It means that love is standing faithfully at someone's side every day knowing that one day they won't be there anymore. Specifically, in the video, the boyfriend is killing himself with his smoking and that's what's tormenting the girlfriend. That's the way I understand it, at least.
